Salary: £21,806 to £23,753 per annum for 32 hours per week
£13.10 up to £14.27 per hour actual
Location: Cardiff & Vale (Hospital Wards, Homes, and Community Settings)

We are launching an exciting new pilot service and are looking for a compassionate and dynamic individual to join our team as a Therapeutic Wellbeing Outreach Worker.
This role is at the heart of our mission to provide seamless, therapeutic support to older people, primarily those living with dementia, as they transition from hospital back into their home or care setting.
As a Therapeutic Wellbeing Outreach Worker, you will be a vital connector between hospital and home. Your primary focus will be to reduce anxiety, build confidence, and support the overall wellbeing of both patients and their carers through a person-centred approach.
Key responsibilities include:
Ward-Based Therapeutic Support
Delivering creative, sensory, and relational wellbeing sessions on hospital wards to prepare patients for discharge.
Community Follow-Up: Providing continuity of care by visiting individuals in their own homes or care settings after they leave the hospital.
Offering emotional support, information, and wellbeing check-ins to families and carers throughout the journey.
Experience supporting older people and/or people with dementia is Essential
