Out and About 50+ Project – Referral Information

Age Connects Cardiff & Vale is launching a new 5-week support programme aimed at helping older adults in the Vale of Glamorgan regain confidence, improve mobility, and reconnect with their communities.

What the service offers:

  • Supported community outings (e.g. shopping trips, short walks, social groups)

  • Recreational activities to encourage independence

  • Support and guidance on using public and personal transport

  • Confidence-building resources and emotional support

Eligibility for referral:

  • Aged 50 + & Living in the Vale of Glamorgan

  • Expresses a desire to regain independence and participate in community activities

  • No cognitive impairment affecting decision-making or safe participation

  • Able to use public or community transport or a taxi service

  • Referrer believes the individual has the potential to go out independently after the 5-week programme

Outcomes we aim to achieve:

  • Improved mobility and confidence

  • Better mental well-being

  • Reduced social isolation
