Health & Social Care Regional Integration Fund Guidance

The Health & Social Care Regional Integration Fund is a five year fund from April 2022-March 2027.  It builds on the progress made under the previous Integrated Care Fund and Transformation Fund. The fund will help integrate health and social care services.
