Bereavement Feedback Questionnaire
We are reaching out to you regarding the Bereavement Feedback Questionnaire developed by the Bereavement Support Service, within Cardiff and Vale University Health Board. We would like to hear about your experiences with bereavement assistance and support in your local area. As part of the Patient Experience Team we want to hear your feedback.
We have a questionnaire which can be answered using the link below or we have paper copies available on request. We would also like to meet with you to discuss your experiences, please do let us know if there are any groups or meetings we might attend to talk about how we can work to improve services in the future.
