Cardiff and Vale Framework for Older People

Meaningful and Purposeful Lives

‘Meaningful and Purposeful Lives’ sets out the framework for Older People’s services for the Cardiff and Vale of Glamorgan Integrated Health and Social Care Partnership for the period 2014-2019. This framework is also suited to those with longer term or complex needs who may not be “older”. It has been designed for the population of Cardiff and the Vale of Glamorgan which is changing with people living much longer with the expectation of many years of healthy retirement. This brings changing requirements for their health, social care and housing needs which public services and the third sector need to respond to in partnership with the people we serve.


Circulating information to the Community Resource Service (CRS) and Community Resource Teams (CRTs)

The CRS in the Vale and CRTs in Cardiff bring together health, social care and voluntary sector staff and provide a range of services to support rehabilitation and help older people remain independent in the community.
